A patient with severe pneumonia involving both lungs is in respiratory failure in the intensive care unit. He has an endotracheal tube in his trachea and is connected to a mechanical ventilator. In an effort to improve his oxygen and carbon dioxide levels, the doctor increases the size of each breath delivered by the ventilator. This results in an increase in the alveolar pressure to very high levels, in some cases to levels greater than the associated capillaries. How will this change in the ventilator affect the matching of ventilation and perfusion in the lung?

In the 1950s, there were several severe polio epidemics in the United States. Many people who were infected with the polio virus developed failure of the respiratory system and were kept alive in an iron lung, an airtight cylinder that encased the patient’s body, except for the head, which protruded from the device. The iron lung was attached to a large vacuum that cycled the pressure inside the iron lung from atmospheric pressure to a negative pressure and then back to atmospheric pressure. This device led to the creation of a negative pressure in the pleural space on each cycle.

For the pectoralis muscle, which normally acts to move the arm, to function as a ventilatory muscle, the arm must be fixed in position. Typically, patients in respiratory distress learn to do this on their own and assume a “tripod position” by resting their hands on their knees or on a nearby table or chair (Fig. 2-4). With the position of the arms secure, contraction of the pectoralis results in elevation of the anterior wall of the chest. Chances are you have also done this at some point after heavy exercise as you bent over with your hands on your knees and tried to catch your breath.
